Born in Jasper, Alberta, Hilworth played with the Drumheller Miners of the AJHL before joining the WCJHL's Medicine Hat Tigers. He was chosen 55th overall by the Red Wings in 1977 after registering 34 points and 268 penalty minutes in his last junior season.
The young rearguard played five games in Motown during the 1977-78 season but saw most of his ice time with the Kansas City Red Wings of the CHL. Hilworth split the next year between Detroit and K.C., but was strictly a farmhand by the next year. After being picked up on waivers by the Edmonton Oilers in March, 1980, he spent a year with the CHL's Wichita Wind. He retired early in the 1984-85 season after a solid 2 1/2 years in the IHL.
